Astrid Adrienne : The ICAO had divided the stage of safety aviation decade in three stages, namely the fragile system (1920-1970), safe system (1970-1990), and ultra safe system (1990 onwards). In fragile system stage, the efforts to increase the flight safety was directed to individual risk management by promoting the intensive training.

The approach was coupled with taking the learning from the results of aircraft accident investigation. At this period, there was one accident per one hundred flight activity.

In a safe system stage, the improvement for the flight safety was carried out through technological and regulatory approaches, more sophisticated aviation technology and strict regulation. In that period, learning was taken from the results of an investigation into the incident, as well as accident investigation. The accident  target was one accident for every ten thousand flights.

In ultrasafe system stage, the approach taken was to increase the safety with the pattern of business management. Learning is not just from the  investigations result of accidents and incidents, but also from observations of daily flights normal operation.  In that period, fatal accidents were  estimated to happen only once in a million flights.

It's very tragic, in between the gap of ICAO ultrasafe program system, with airport, ATC and the airlines should gradually implement the SMS (safety management system), Indonesia was faced with the tragic reality of commercial transport aircraft "Sukhoi Superjet 100 (SSJ100)".

 In the aviation world, we should not ask, " Who is wrong?". It's a very uneffective criminal question in an effort to increase the aviation safety. The real question is,  "What is wrong?". When there were indications of human error in an accident and the investigation actually has just started, what is questionable is, "What is wrong in aviation system?"

The human error is a consequence of poor system and the system is never perfect because of various conflicts and contradictions.

In the ICAO's safe system stage, it was said that during the period 1970-1990, the approach taken was to investigate the incident. The approach was based on the theory which said it could happen one fatal accident of 600 incidents.

Implementation of incident investigation was also very closely related to the culture. ICAO divided countries into three phases of aviation culture: pathological, bureaucratic, and generative. Implementation for the mandatory reporting and investigation of incidents could not be implemented in countries that were still at the level of pathological and bureaucracy.  In pathological countries, the incident report was always hidden and the failures were always covered. In the bureaucracy countries,  the incident reports were ignored and the failure was excused without research. In the generative countries, the incident reports were always sought after and the failures were scrutinized for later repair.

One of the things that must be considered in flight is the situational awareness, such as threat factors that should be the pilot's attention. ICAO has revealed several factors : passengers, terrain, ATC, call sign, time pressure, flight diversion, system malfunction, missed approach, automation, airport, heavy traffic, ground crew, maintenance, weather, cabin crew, and distraction.

Such factors must be considered by the pilot. Consequences and risks were calculated, and the necessary actions taken so that these factors didn't form a human error.

The erroneous threat management will lead to human error, the human error and improper management will lead to undesired state of aircraft. Improper management of aircraft movement is one that will cause an accident or incident.

In SSJ100 flight, the threat which should be realized by pilot is the terrain (mountains, hills, which are not well known by the pilot), heavy traffic, time pressure (the pressure to do the demo many times), weather and system malfunction (an early warning system or a ground proximity warning system / GPWS when flying too low , that maybe it does not work).

Didn't recognize the terrain (mountain, hill) would will lead to human error such a request to go down to an altitude of 6,000 feet. If human error was unproperly managed (uncorrected by the peers), then there was a false movement of the aircraft (down to an altitude of 6,000 feet). If the motion was untrue, unproperly corrected through the GPWS (Ground Proximity Warning System), then came the tragedy : the plane crashed into a mountain.

In ICAO's Human Factors document about Organization and Management was disclosed about the case of DC-10 that crashed into Mount Erebus in the Antarctic. The plane was in flight tour (joy flight) carrying passengers who wanted to see the mountains in south pole. The investigation revealed that one of the factors that contributed to the accident was management's decision to put the captain of the pilot and copilot who both had never flown to the area, so that when they were trapped by the Whiteout phenomenon, the pilot didn't realize the plane's direction and then crashed into a mountain.

So, why did the SSJ100 crash into a mountain? Of course, the NTSC (The National Transportation Safety Committee) will give its answer later. Hopefully the accident could be a lesson in the future.

Since the publication of books in circulation up to Pakistan, Turkey, Iraq, and India, Manji received several death threats from around the world. In her book, Manji showed the evil of individual Muslims throughout history, hypocrisy ruler, and the boards that called rise of Islam, such as the position of the oppressed Muslim women, and cunning strategy of Saudi Arabia that dominated the Muslim states.

She also wrote "Islamic Reform Now". In the book, she described the most fundamental weaknesses of Islam, such as terrorism in the name of religion, excessive hatred toward other people, and the cult of Koran.

Astrid Adrienne :  Myint Oo's resignation is believed to mark the end of the existence of extremist groups, especially from the military, in the administration of President Thein Sein. Myint Oo (retired general) is known as a leader in the hard-liners in Myanmar. He is also known to be a confidant of former strongman of the ruling regime as well as past military junta, Than Shwe, who resigned from the political map of Myanmar a few months before the November 2010 general election. Myint Oo's career began in the military since he graduated from military academies of the country in 1970. In the 1990s, he served as military commander who served in the Myanmar border area with China. He was promoted to First Secretary of the military government in 1997. Furthermore, in 2009, Myint Oo was promoted to serve as military advisor to the Senior General Than Shwe. A year later, he was elected as a member of parliament for military support/Union Solidarity and Development Party (USDP) for later appointed as vice president by a military delegation.

  Astrid Adrienne : This is a picture of an apple apples. To be able to eat apples, you can't just look at it or download pictures of apples, but you have to go get a real apple. Every once a week, I always buy more than four kilograms of apples for my snack food within a week. Adriaan Zef --- my husband, my buddy, my boy friend, my beloved beloved  partner --- is very fond of Apple juice.


This actually has nothing to do with a best time or a certain period to consume apples, but these days Zef often speaks to me with tones such as reading the book aloud, or, rather, he is like an officer of the announcement. He said with a half-spelled words, "Apple - Benefits -  for- Pregnant - Women (You don't have to read aloud, BB.I'm not deaf and I love you,too, ... but of course, it is a subject!.... ) : lower cholesterol, prevent cancer and nourish the lungs, preventing heart disease and stroke, can reduce the weight of the human body, maintaining the health of teeth and protect the body of influenza virus." Then as a modern man who had just his first time to learn about the apples benefits, he took the initiative, "Honey, if so, we have to buy apples as much as possible for our baby."

See? I've bought so many apples first, but he thought that it was his idea.

 Lakshmi Lavanya : C, one of my colleagues in Indonesia reported that the body of Sukhoi Superjet 100's pilot, Alexander Yablontsev was eventually found. This is the only bodies of victims found in the intact condition. I commiserate with the victims suffering, my mother cried when she read the newspapers and watched the news events through TV broadcasts in our country.

 Jan Pepijn Servaas : That's good news, Lakshmi. I hope the identification process can be done more quickly. What about the media coverage there? Do they consider the victim's family's feeling in journalistic reporting?

 Lakshmi Lavanya : Our TV broadcast the incident news only at certain hours, I do not know exactly how the incident's report through the media in Indonesia. Thank you for asking, Jan. Though I'm not the press member, but I guess I can understand the intent of your question.

 Jan Pepijn Servaas : Well, as you know,... it is very possible and often happens, there is massive coverage by the mass media and the journalists do not comply the journalism ethics code.

Fairuz Azalia : C, I think the Muslims need to be vigilant with God, Liberty, and Love (Irshad Manji's book), because if you look further, it is a part of conditioning movement to pioneer the legalization of same-sex marriage laws, men married to men and women married to women. Am I wrong if I do not agree with all the awkwardness?
